Thank you for booking your first appointment, we look forward to meeting with you. After your appointment is confirmed you will receive a confirmation email with all new patient paperwork attached. We ask that you fill out your paperwork using this link at least 24 hours prior to your first appointment so that the doctor can review it. If for any reason you can not fill out the paperwork online please let us know that you will be downloading the forms online to fill out or coming in prior to fill out paperwork. This allows us to review the financial policy and confirm your appointment.

Financial Agreement and Cancellation Policy
Notice of Cancellation Policy:
Your appointment is very important to me. Our new patient intake is an hour and half long and that time is set aside to get to know you and your health care needs. We ask that you please give us at least 24 hours advance notice if you need to reschedule your appointment. Giving at least a 48 hour notice of the need to cancel is ideal. We require all patients to have an active card on file. This card will be used for the sole purpose of enforcing the cancellation policy as outlined in your new patient paperwork.
