Notice of Cancellation Policy:
Your appointment is very important to me. Our new patient intake is an hour and half long and that time is set aside to get to know you and your health care needs. We ask that you please give us at least 24 hours advance notice if you need to reschedule your appointment. Giving at least a 48 hour notice of the need to cancel is ideal. Under most circumstances, appointments that are rescheduled within 24 hours of their scheduled time will be assess a 50% charge of the appointment. Any patients who do not call or show up for their appointment will be charged 100% of the appointment cost. If there is an emergency please call the office to discuss and reasonable accommodations will be made.
In order to enforce this policy at the time of your initial appointment you will be sent an invoice for a $1.00 appointment deposit and asked to keep your card on file. This card will be used for the sole purpose of enforcing the cancellation policies as outlined above and as part of the financial agreement.
